To help reduce the number of homeless dogs and cats, we run a Last Litter Program. We accept entire litters up to eight weeks old, subject to availability in the program.

We will arrange for the mother to be desexed at Condamine Veterinary Clinic, Warwick — free for pension or health care card holders, or a co-payment of $100. Microchipping and vaccination of the mother is also recommended, at your cost.

Complete the surrender application below to see if a space is available. Preference is given to pension or health care card holders.

PLEASE PROVIDE THE FOLLOWING:

A current photo of the mother dog or cat.

A photo of the father, if known.

A photo of all the puppies/kittens born to the mother.

Any certificates i.e., desexing, vaccinations.

If microchipped, the microchip details and a signed transfer of registration form.

Details of any worming completed, including date and brand of the wormer.

A copy of your pension concession card or health care card.
